Team Check In CANNOT be done prior to your game.

Please scan and email the following information to CESAcheckin@gmail.com by Thursday, September 15, 2022.

The email Subject line needs to be your team's Official Name and Age Group. ALL DOCUMENTS should be submitted at one time, If you don't have all required documents ready DO NOT emaill until all are complete.

Required Documents:

1. Completed Tournament Checklist Form. Click here for form.
2. Completed 2022 St. Francis Sports Medicine CESA Classic Player Liability Form.
3. Official State Certified Tournament Roster OR Official State Certified Team Roster OR Official State Certified Travel Roster
4. Permission to Travel Documents (For out of State Teams Only). Contact your club registrar for required paperwork.
5. Guest Player Roster (If Applicable)
6. Player card copy of all team members. Include Coaches and Players.

​Player card for each player, guest player(s) and coach(es) will be checked at the fields prior to each game. Only Official State stamped laminated player cards or Official Digital Player Card System will be accepted.
